Regina Berko, founder of EverGrace Care, is a hospital Registered Social Worker with years of experience in hospital discharge planning and complex case management. She spent that time sitting across from families during some of the hardest weeks of their lives — helping them figure out who would help at home, whether the house was safe to return to, and who would notice if something went wrong in those first uncertain days. Again and again, she saw the same gap: hospitals are built to treat illness and injury, but the weeks and months that follow are often left for families to figure out on their own, with little guidance and even less support.

That gap became personal when Regina's own father was diagnosed with cancer. Suddenly, she wasn't the social worker across the table anymore. She was the daughter, trying to help her parents navigate a system she knew professionally but had never lived through personally. She watched her mother take on the caregiving almost entirely by herself: managing medications, coordinating appointments, losing sleep, and slowly losing pieces of herself in the process. Regina had the professional knowledge to help, and even then, it was hard. She often found herself thinking about the families who didn't have that knowledge who were figuring it all out from nothing, at the worst possible time.
